Choosing Medicare coverage is not just a benefits decision. It is a provider access decision. In Gainesville, where two major health systems — UF Health Shands and HCA Florida North Florida Hospital — serve the region, the coverage path you choose directly affects which facilities and specialists you can use, and at what cost.

Two Paths: Original Medicare and Medicare Advantage

Original Medicare includes Part A (hospital coverage) and Part B (medical coverage). It does not restrict you to a network. Any provider who accepts Medicare — anywhere in the country — is available to you. Most physicians and hospitals in the Gainesville area accept Original Medicare, including UF Health Shands and HCA Florida North Florida Hospital.

Medicare Advantage (Part C) is an alternative offered through private insurers. These plans provide the same Part A and Part B benefits but typically operate through provider networks. Depending on the plan type, your access to local providers may be limited.

HMO vs PPO Plans in Gainesville

Medicare Advantage plans in Alachua County include both HMO and PPO options. The difference matters.

  • An HMO (Health Maintenance Organization) generally requires you to use in-network providers and obtain referrals to see specialists. If your preferred physician or specialist is not in the plan’s network, you may pay the full cost out of pocket.
  • A PPO (Preferred Provider Organization) allows you to see out-of-network providers, but at a higher cost. This offers more flexibility — though it does not eliminate network considerations entirely.

UF Health Shands is an academic medical center with a large network of specialists. Specialist availability within specific Medicare Advantage plans can vary — a plan may include UF Health as a system while specific physicians bill separately or are not individually contracted. Before enrolling, verify that your specific UF Health providers are included in the plan’s network, not just the system name. Networks change annually.

Adding Medigap to Original Medicare

Some Gainesville-area beneficiaries pair Original Medicare with a Medicare Supplement Insurance Policy, also called Medigap. Medigap does not use provider networks. If a provider accepts Medicare, they accept your Medigap plan. This is particularly relevant for patients with ongoing specialist relationships at UF Health who want to avoid network restrictions.

For a detailed comparison of these two approaches, see Medigap vs. Medicare Advantage.

Prescription Drug Coverage

Original Medicare does not include drug coverage. A standalone Part D Prescription Drug Plan is needed. Medicare Advantage plans often include drug coverage. Formularies — the list of covered drugs — and pharmacy networks vary by plan. Confirm your medications and preferred pharmacy before enrolling.

Verify Before You Enroll

Provider networks, formularies, and costs change on January 1 each year. Do not assume last year’s network applies to the current plan year.
